The Somerset West HART Hub is a low-barrier, integrated health and social care hub for people with complex health, mental health, substance use, and housing needs.
The HART Hub brings together primary care, mental health services, substance use supports, housing coordination, and system navigation in one location. Our focus is on reducing barriers, improving access, and supporting people toward stability, recovery, and housing.
