What is the HART Hub?
The Somerset West HART Hub is a low-barrier, integrated health and social care hub for people with complex health, mental health, substance use, and housing needs.
Drop-In Centre

Open daily from 9:30 a.m. -10:00 p.m.
(Thursdays, 12:00 p.m. - 10:00 p.m.)​ -
Snacks, tea & coffee available all day

Shower, hygiene supplies & clothes

Fun activities (movies, crafts, bingo)

Referrals to HART Hub services and external agencies

Peer led support groups and activities
System Navigation
Offering social service and healthcare navigation throughout Ottawa, including:

Care coordination
-
Crisis support
-
Obtaining or replacing ID
-
Referrals to harm reduction, rapid pathways to withdrawal management , substance use treatment, mental health supports, housing programs, supportive housing, etc.

Advocacy, community resource provision

Primary Health Care Clinic
The HART Hub’s primary care team provides walk-in and ongoing primary care services for people with complex health, mental health, substance use, and housing needs. In addition to episodic care, clients may be rostered for continued primary care support as appropriate.

Wound care

Assessment and treatment of infections, including antibiotics

Laboratory requisitions and diagnostic imaging referrals

Walk-in clinic services for acute or urgent concerns
-
Registered Nurses available daily during drop-in hours
-
Nurse Practitioner available Monday–Friday

Peer Outreach

Offering client support in the SWCHC catchment area, including:

Harm reduction supplies, including Injection/smoking gear, sexual health supplies, and narcan

Safe needle disposal

Mental health and crisis support

Hygiene supplies, clothing

Food and beverages
-
Overdose education and response
-
Daily community clean up with honoraria provided to client participants
