MULTI-YEAR SERVICE ACCOUNTABILITY AGREEMENT (M-SAA)
In April 2014, Somerset West Community Health Centre (SWCHC) entered into a Multi-year Service Accountability Agreement (M-SAA) with the Champlain Local Health Integrated Network (LHIN). The M-SAA agreement includes a number of performance standards that SWCHC has to meet.
Our progress towards meeting the performance standards set out in the M-SAA is outlined in our Balanced Scorecard.
QUALITY IMPROVEMENT PLAN (QIP)
SWCHC is committed to improving the quality of care we provide to our clients. That's why we produce a Quality Improvement Plan (QIP) to map out how we will address quality issues and meet our quality improvement goals.
