1. What is the Somerset West HART Hub?
The Somerset West HART Hub is a low-barrier, integrated health and social care hub for people with complex health, mental health, substance use, and housing needs.

​The HART Hub brings together primary care, mental health services, substance use supports, housing coordination, and system navigation in one location. Our focus is on reducing barriers, improving access, and supporting people toward stability, recovery, and housing.

 

2. Who is the Somerset West HART Hub for?  
The Somerset West HART Hub prioritizes individuals experiencing the highest barriers to care, particularly those facing homelessness, substance use challenges, and complex mental health needs. While rooted in the Somerset West community and responsive to those impacted by the CTS transition, the HART Hub operates as a regional, needs-based program. Access is guided by complexity and vulnerability, not catchment boundaries.

3. What are the HART Hub’s drop-in hours of operation?​
Hours are designed to be accessible and responsive to community need. 

The Somerset West HART Hub offers drop-in services during the following hours:

  • Monday–Wednesday & Friday: 9:30 a.m. – 10:00 p.m. 

  • Thursday: Opens at 12:00 p.m. until 10:00 pm

  • Saturday & Sunday: 9:30 a.m. – 7:00 p.m

4. Do clients need a referral or appointment?

There is no wrong door.

 

Clients can walk in or be referred by community partners, hospitals, shelters, or emergency services. Access remains flexible while balancing capacity and acuity.

5. What services are offered at the HART Hub?

Care is co-located, coordinated, and person-centered. Services include:

  • Primary care and nursing services

  • Mental health and counselling supports

  • Substance use assessment and treatment coordination

  • Health system navigation and care coordination

  • Housing support and pathways to supportive housing
     

The HART Hub provides place-based care, not mobile outreach.

6. Does the HART Hub provide withdrawal management or residential treatment?

We provide rapid access and supported referrals to withdrawal management and residential treatment through trusted community partners.

 

The HART Hub does not provide on-site withdrawal management or residential treatment services. However, we maintain a rapid referral pathway with Montfort Renaissance.

 

While we support rapid assessment, coordination, and follow-up, placement is based on availability and clinical criteria. Beds are not reserved for the HART Hub and are offered by partner organizations on a first-come, first-served basis. Our team works closely with individuals throughout the referral process to maximize timely access and continuity of care.

7. Are outreach services part of the HART Hub?

Outreach and the HART Hub are distinct but complementary.

 

Outreach services are not part of the HART Hub. Outreach teams work alongside clients in the community, meeting people where they are and supporting engagement. The HART Hub provides a place-based option that outreach teams can connect clients to when appropriate.

8. How does the Somerset West HART Hub work with other HART Hubs in Ottawa?

HART Hubs are locally designed and regionally coordinated.

 

While every HART Hub reflects local community needs, the Somerset West HART Hub works closely with the Ottawa West HART Hub to complement services and coordinate care where appropriate, avoiding duplication and strengthening system pathways.

9. What values guide the Somerset West HART Hub?

Care is grounded in dignity, respect, and relationship.

 

Our work is guided by:

  • Trauma-informed and culturally responsive care

  • Client choice and self-determination

  • Equity, inclusion, and anti-stigma practice

  • Collaboration and shared accountability

 

 

10. Do clients need a health card to access services at the HART Hub?

Lack of identification should never be a barrier to care.

​

No, clients do not need a health card to access services at the Somerset West HART Hub.

  • Many people we serve have lost identification or face barriers to maintaining documentation. One of the roles of our system navigators is to help clients re-establish their Ontario health card and other identification as part of broader care coordination and stabilization.

  • HART Hub services are designed for individuals who are eligible or could qualify for an Ontario health card. Our team works with clients to address documentation gaps so they can reconnect with the health system and access ongoing care.

 

 

11. Do I have to be abstinent to access care at the HART Hub?

Recovery looks different for everyone.
 

No, clients do not need to be abstinent to access care at the Somerset West HART Hub.
 

We recognize that recovery can mean many different things to different people, and that change happens over time. Our approach is to meet people where they are at and work alongside them to support their individual health and wellness goals.

While substance use is not permitted on site, we provide care, support, and coordinated services that help people move toward greater stability, health, and recovery — at their own pace.

12. How do partners or community members contact the HART Hub?

As of February 1, 2026, the Somerset West HART Hub will begin accepting community referrals.

 

Partners and community members are encouraged to call the HART Hub reception to discuss a client’s needs and determine the most appropriate next steps.

Please note that housing placements are limited, and referrals are prioritized based on acuity, readiness, and available capacity.
