Health Services
PRIMARY HEALTH CARE SERVICES
Our dedicated team of health care professionals work with and for clients to ensure the best care possible. Health promotion is integrated in all programs and services. Our common goal is to support individuals and groups gain the knowledge, skills and self-confidence to access resources and make healthy choices. Our team includes: family physicians, nurse practitioners, nurses, lactation consultant, acupuncturist, chiropodist, dieticians, lung health educator, and a health promoter.
Medical Clinic
Our focus is to optimize health, and ensure the best possible care in times of illness.
Our medical clinic, staffed with highly skilled family physicians working collaboratively with other health professionals, is open Monday to Friday with extended hours three evening a week. The Health Services team offers comprehensive primary care: preventative health and information, assessments, health management and referrals. Sensitive to the needs of newcomers to Canada, our services are offered in English, French, Cantonese, Mandarin, Vietnamese, and Russian.
Walk-in Services
Our Walk-In Clinic provides urgent care to clients of the Centre and those without a health card or permanent address. Services are also available to those who live in our neighbourhood who cannot get an appointment with their regular family physician. Nurse Practitioners provide assessments and appropriate treatment.
Healthcare Out in Our Community
In response to the needs and concerns in our community, our health professionals provide outreach to individuals and groups. They visit shelters, drop-in centres, seniors’ buildings and homes – wherever the need arises.
Outreach health professionals work in the community to connect individuals and families with the information and resources they need to take good care of themselves – physically, mentally and socially. Working in collaboration with our colleagues in Community & Social Services, Ottawa Public Health, other Community Health Centres and community partners the multicultural nurses provide culturally appropriate services in many languages throughout our community.
After-Hours Emergency Care
Clients who are registered with our medical clinic can access 24-hour on call care through Somerset West Community Health Centre. This service is designed to respond to sudden, unexpected illness that cannot wait until the next day for treatment.

- Foot care services
- Breastfeeding information and support
- Obstetrical care and perinatal assessments
- Smoking Cessation
- Flu immunization clinics - at SWCHC or in the community
- Health Education workshops in the community
