CHANGE A LIFE, TRANSFORM A COMMUNITY'S FUTURE
When you become a partner with the Somerset West Community Health Centre, your support changes the lives of preschoolers and seniors, newly arrived immigrants and those with disabilities, youth and those facing discrimination – but it also creates the hope of a brighter future for an entire community.
When people come together to work through their problems, identify solutions and bring about needed change, a stronger, healthier community is born. You can play a key role in that transformation through your support. Here are just a few examples:
| $750.00 | Will cover the cost of transportation to and from recreational programs for 20 isolated seniors per year. |

| $500.00 | Subsidizes the participation of 10 student's in an after-school program or homework club and give him or her support needed to reach their full potential academically and socially. |
| $350.00 | Funds 4 HIV-prevention sessions for youth in the community, preventing needless spread of this life-threatening disease. |
| $150.00 | Sponsors a youth's participation in a leadership development program, enabling him or her to become a mentor and positively influence other youth in the community. |
| $50.00 | Provides a new toy for our toy library or for a community playgroup, aiding the cognitive development of infants and young children. |
